I [employee name] acknowledge receipt of 1 X headset and understand that the headset is for my own personal use, and is my personal responsibility to ensure I have it to use on shift every day.
In the event that the headset is lost I understand there is a £60 replacement charge, which will be deducted from my monthly salary should it need to be replaced.
